It's by design. If you get a call, Name & Phone Number will display the inbound Caller ID Name and Number.
If you dial a number, Caller ID doesn't resolve the Caller ID Name of the number you've dialed. It's just the number you dialed.
If you hover your mouse over an inbound call, its alt tag will display the number.
If you download your Call History as an .csv file, the inbound Caller ID Name and Number and combined in the same cell.

I don't see the phone number or the name I only see either wireless caller or the city.

Hover your cursor over "Wireless Caller" and the number will display as a pop-up. If not, try another browser.

I think the problem is OP is using a phone for everything. No cursor with a phone.
